iHealthConsumer sends out the 5498 SA in the second week of May. This enables reporting of any prior year contributions which are made up to the current year April 15th deadline.
The 5498 SA identifies the custodian of the iHealthConsumer account. You would see the custodian listed under TRUSTEE as PNC Global Investment Servicing. It would identify the account holder.
